Diabetes Health Education Program

Added Benefit for Members Who Have Diabetes

If you have diabetes, managing the disease will be a significant part of your daily routine until there is a cure. This process involves you, those close to you, your physician and other caregivers.

Arkansas Blue Cross and Blue Shield, Health Advantage and those BlueAdvantage Administrators of Arkansas employers who purchase the program offer enrolled members a free community-based Diabetes Education Program to furnish information about managing this disease. This information should help you:

  • Prevent complications common with diabetes;
  • Quickly and effectively resolve acute episodes of illness associated with your diabetes.
Program Benefits
  • Registered nurse case managers who can assist with your health plan benefits;
  • Information and education from community and national resources, which follow the guidelines set forth by the American Diabetes Association;
  • Information to the physician who is developing a plan of care for your diabetes.

Program Highlights

  • Voluntary Health Survey: A comprehensive telephonic questionnaire to help determine your current health status and identify your health education needs. After you complete the survey, a personalized report will be mailed to you.
     
  • Member Contact: Local one-on-one contact with a registered nurse case manager to assist in directing you to local and national resources. Our case managers can assist you with your health-plan benefits.
     
  • Physician Collaboration: Working with you and your physician to provide diabetes information using the American Diabetes Association Standards of Care and your physician's care plan.
     
  • Health-Care Resource List: Comprehensive list to help members locate community resources and educational classes for access to diabetes care and education.
     
  • Certified Diabetes Education Programs: Connecting you with health programs offered in your community.
     
  • Diabetes Control Network (DCN): The network provides information to educate, motivate and support members with diabetes. It offers a series of bilingual diabetes newsletters and selected health brochures based on the American Diabetes Association Standards of Care for diabetes. When you enroll in the Diabetes Health Education Program, you will automatically be enrolled in the DCN. You also may enroll in the DCN by calling 1-800-326-5554.
     
  • Satisfaction Surveys: For you to complete to keep us informed on how the program is working for you.

Physician Support

The physician remains your caregiver and makes all medical decisions. The Diabetes Education Program provides only health education assistance to the physician in managing a member's disease through education and benefits management and facilitates access to local and national resources.

Health/Cost Advantages

If you are dealing with the effects of diabetes, you know firsthand what the cost is to your health and quality of life. The financial costs are another issue. Better health leads to a better quality of life and lower costs to you. Please check your member benefits document for coverage of services.

How To Enroll

Call a registered nurse case manager at your nearest regional office to enroll in the Diabetes Education Program today or download the Patient Enrollment Form (101 KB PDF) and send it to your regional office. The program is completely voluntary, and you may choose to leave the program at any time.
