If you obtain routine care from out-of-plan providers neither Medicare nor
Medi-Pak Advantage will be responsible for the costs.
Eligibility
Medi-Pak Advantage is open to all Medicare beneficiaries eligible by age or disability in the plan’s services area (Arkansas).
Benefit and Plan Premium Information
You must continue to pay your Medicare Part B premium even if your Medi-Pak
Advantage premium is $0.

Medi-Pak Advantage’s contract with CMS is renewed annually and that the availability of coverage beyond the end of the current year is not guaranteed.

Benefit Changes
Medi-Pak Advantage benefits, premium and/or copayments/coinsurance may change on January 1, 2009.
